Patient Information

All medical information is kept in the patient file, and some information is held on the computer. From time to time, we undertake audits with other Departments within the Health Board, to make sure we are providing the best care for our patient’s. An emergency care summary is held on a confidential database and is designed to aid Accident and Emergency, and Out of Hours if required with a summary of all your medication and allergies.

Disabled Patients

A large print version and an audio version of the Practice Leaflet are available on request.

The medical centre has disabled access throughout the building. The toilet facilities are designed for partially or fully disabled people.

A loop system is in place at the reception desk and in the waiting room.

Data Protection

The Practice is registered with the Information Commissioner, who holds a description of the data collected and retained by the Practice. Our nominated Data Controller is the Practice Manager.

You may sometimes be asked to sign a consent form by a Solicitor/Insurance Company before we will release any information about you from your GP.

Freedom of Information

Practice Information can be requested under the Freedom of Information Act. The Practice is willing to provide you with a copy of the policy at any time and requests for information need to be put in writing to the practice manager.

Violence

Our surgery is not obliged to deal with anyone who is violent or threatening, and these people will be asked to leave the premises.

Meetings

The Surgery holds monthly Clinical meetings, whereby the District Nurses, Community Psychiatric Nurses, Social Worker, Health Visitor, MacMillan Cancer Support Nurse and GP’s discuss patient’s care. These meetings are also open to anyone the practice feels it would be appropriate for them to attend.  Please be assured that all discussions are confidential.

The admin staff hold meetings wherever possible.  These were originally held at some in-house training sessions, however, due to these being reduced in number, it has been agreed the practice will hold staff meetings on some bank holidays when NHS24 will cover.

Patient group meetings are held twice yearly to discuss how the practice is run from a patient’s perspective. If you are interested in attending these meetings, please contact the Practice Manager, or complete the form on this website.

Complaints/Information and Suggestions

Complaints most frequently arise from misunderstanding or lack of communication. We operate a practice complaints procedure which the receptionists or Practice Manager will be happy to discuss with you. Our aim is to give the highest possible standard of service and to deal swiftly with any problems that may occur. Suggestions are always welcome, and a suggestion box is provided for patient’s comments in the waiting room.

Training

The Surgery is involved in continuous appropriate staff training, and holds approximately nine half-day training sessions annually when the Surgery will be closed. All other training is on an individual basis.

Chaperoning

All patients are entitled to have someone present for any consultation, examination or procedure. If you require further details please ask at reception for a leaflet.  The practice also has a policy in place if a chaperone is required by the doctor during a consultation.

Young People

Young people can often feel embarrassed about coming to see a doctor or nurse. We would like to encourage you to feel at ease when you come to the surgery and feel confident when you are talking to your doctor or nurse. When you help us to make a decision about your treatment, we need to know you fully understand it. Whatever is discussed at the surgery is confidential as far as all surgery staff are concerned, however, you are allowed to talk about your visit with whoever you like, and we would encourage you to talk with your family about what has been discussed. We can discuss relationship issues with your doctor or nurse. Health issues like contraception, sexual health, and lifestyle issues like smoking addictions and other social issues. You do not have to tell the reception staff why you want the appointment, and they will not ask you any questions.
